Safari 5.1, Safari 6: Unable to access websites via web proxy

After installing Mountain Lion (which includes Safari 6), or installing Safari 5.1 or Safari 6 in OS X Lion, you may not be able to access websites via a web proxy (HTTP) that uses a file-based Proxy Automatic Configuration (PAC) file.
